To make sure that your treatment is appropriate to you, all new clients are required to complete a health consultation form. In some cases, treatments may need to be adapted, or an alternative may be recommended, depending on individual health considerations. I reserve the right to decline a treatment where contraindications are present. 

Health forms will be completed upon first appointments, so I kindly ask new clients to arrive 15 minutes early to ensure your full treatment time can be enjoyed. If you are currently taking any medication, it would be helpful to have details available to include on your form. Returning clients will simply be asked to confirm any changes to their health at each visit.
